Membership is open to companies, institutions and individuals who wish to support hospice and palliative care concepts through affiliation with AHPCO.  We have a variety of categories of membership and members enjoy valuable benefits -Learn More.

Member Categories

Hospice Provider

Census-Based**
An organization licensed as a hospice by the Arizona Department of Health Services and operating a hospice program consistent with the current Standards and Principles of the National Hospice and Palliative Care Organization.  
**$3.00 per unduplicated patient admission in previous calendar year.  (Minimum = $100 Maximum = $5,500)

 

Hospice Provisional Provider

Complimentary until a calendar year of serving patients has elapsed.
An organization newly licensed as a hospice by the Arizona Department of Health Services and operating a hospice program consistent with the current Standards and Principles of the National Hospice and Palliative Care Organization. The provisional membership is valid for up to a year and then will be converted to a Provider Member upon payment of the current year’s dues.

Palliative Care Provider

$250/year
An entity or individual provider currently providing palliative care as defined in the AHPCO bylaws and not hospice services. 

 

 

Individual

$25/year
Open to individuals who desire to support hospice and palliative care concepts through affiliation with AHPCO. Individuals may not use their membership to solicit other members or otherwise promote any products or services.  

 

 

Associate Members

$300/year
An institution, company or organization, other than a hospice or palliative care provider, desiring to promote hospice and palliative care concepts through affiliation with AHPCO.

  • Member publications and e-communications plus the online Member Forum.
  • Eligible** to serve on the Board of Directors and has voting rights***.

*All employees are eligible for the Member price for educational events - please note that some events have attendee restrictions.
**Limited to one voting member per provider membership.
***Provisional members do not have this benefit until they convert to a Provider membership.
